Job summary

Hopscotch Primary Care in North Carolina is seeking a Primary Care Physician to lead clinic teams and deliver high-quality care. You will supervise the care team, participate in care coordination, and promote the Hopscotch care model in daily practice.

The ideal candidate has 2+ years post-residency, board certification in Internal or Family Medicine, an active NC medical license, and authorization to work in the US. Telehealth, value-based care, and community engagement are part of the role.

Qualifications

• 2+ years of post-Residency experience.
• Board Certified in Internal Medicine or Family Medicine, with an active NC state medical license.
• Authorized to work in the United States.

Responsibilities

• Provide high-quality primary care and leadership for the clinic teams.
• Lead Daily Huddles and coordinate with the care team to improve outcomes.
• Deliver care through in-person and telehealth visits, including Wellness visits.
• Champion the Hopscotch care model, quality, and compliance across the clinic.
• Drive continuous improvement and share best practices to enhance patient care.
