Responsibilities

• Provide day-to-day leadership for clinic teams and deliver high-quality care.
• Coordinate care across physicians and the care continuum.
• Deliver office and telehealth visits, including wellness visits.
• Develop care plans, educate patients, and ensure positive outcomes.
• Lead and participate in clinical forums and team huddles.
• Support operational excellence and integration of new care processes.
