Job summary

Hopscotch Primary Care is seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to deliver comprehensive primary care in a rural North Carolina community. You will manage a defined panel with office and telehealth visits, working with an interdisciplinary team to improve patient outcomes.

Requirements include at least three years in a primary care specialty, active medical licensure, and experience with ICD-10/HCC documentation and EHR systems.

Qualifications

• Minimum of three years’ experience in a primary care specialty (internal medicine, geriatrics, family medicine)
• Experience with HCC documentation and ICD-10 coding
• Experience with multiple software platforms and data management
• Active, non-probationary state medical license

Responsibilities

• Deliver comprehensive primary care services to a set panel of patients, including office visits and telehealth visits
• Work closely with physicians, community health workers, case managers, pharmacists, and others to deliver high-quality outcomes
• Completion of comprehensive annual wellness visits and risk assessments
• Practice with a continuous-improvement mindset, following evidence-based guidelines
• Meet high benchmarks for quality metrics and patient experience
