Job summary

Nashville CARES in Clarksville, TN is seeking a Rural Case Manager – Housing & Support Services to help individuals access housing, healthcare, transportation, nutrition, and finances in rural Middle Tennessee. This full-time, field-based position covers Clarksville, Springfield and surrounding counties.

You will conduct assessments, develop care plans, coordinate with housing and health partners, travel across counties, and document thoroughly.

Qualifications

• Bachelor’s degree required; degree in Social Work, Human Services, or a related field preferred.
• Master’s degree preferred.
• Minimum of three years of experience in community-based social services, healthcare, case management, or a related field.

Responsibilities

• Assess clients’ circumstances, needs, and barriers and connect them with services.
• Provide ongoing case management, including assessments, care planning, eligibility support, and connection to medical and community resources.
• Help clients access housing, healthcare, transportation, nutrition, financial assistance, healthcare, and other community resources.
• Assist clients with locating housing or maintaining their current housing.
• Help clients navigate multiple systems and program requirements while identifying practical steps toward their goals.
• Travel across rural counties to complete home and community visits.
