Job summary

Nashville CARES is seeking a dedicated Field-based Case Manager to help individuals in rural Middle Tennessee access housing, nutrition, transportation, healthcare, and community resources. You will oversee HOPWA and Tenant-Based Rental Assistance programs across Clarksville, Springfield, and surrounding counties.

The role requires strong organization, independent work, and the ability to build relationships with diverse clients while maintaining documentation and program requirements.

Qualifications

• Bachelor’s degree required; Social Work or related field preferred.
• Minimum of three years in community-based social services, healthcare, case management, or related field.
• Strong organizational, communication, documentation, and problem-solving skills.
• Proficiency with Microsoft Office; electronic health record training provided.
• Valid driver’s license, reliable vehicle, and automobile liability insurance for required client and agency travel.
• Ability and willingness to travel throughout the rural service area and to Nashville approximately once per week.

Responsibilities

• Case Management & Client Support: Assess clients’ circumstances, connect to services, provide ongoing case management, and help access housing, nutrition, transportation, financial assistance, healthcare, and community resources.
• Housing & Program Support: Manage client eligibility and documentation for HOPWA-funded services; support Tenant-Based Rental Assistance; maintain documentation and eligibility records.
• Independent Field & Community Work: Manage schedule, caseload, and documentation; conduct client visits; plan travel; coordinate with Nashville CARES staff and partners.

• Travel to Nashville CARES office about one day per week for team collaboration and client needs.

Job description

You’ll play an important role in helping individuals throughout rural Middle Tennessee access the healthcare, housing, transportation, nutrition, financial assistance, and community resources they need.

This is a highly independent, community-based role serving clients primarily in the Clarksville and Springfield areas, as well as Montgomery, Houston, Humphreys, Stewart, and Robertson Counties. You’ll provide comprehensive case management while also overseeing specialized HOPWA and Tenant-Based Rental Assistance requirements for clients in the region.

We’re looking for someone who is highly organized, resourceful, comfortable working independently, and able to build strong relationships with people from a wide range of backgrounds. The right person will be comfortable balancing meaningful client interaction with detailed documentation, program requirements, travel, and problem-solving.

Job Responsibilities

What You’ll Do

Case Management & Client Support

• Assess each client’s circumstances, strengths, needs, and barriers and connect them with appropriate services
• Provide ongoing case management, including assessments, care planning, eligibility support, and connection to medical and community resources
• Help clients access services such as housing, nutrition, transportation, financial assistance, healthcare, and other community resources
• Assist clients with locating housing or maintaining their current housing
• Help clients navigate multiple systems and program requirements while identifying practical steps toward their goals
• Build trusting relationships with clients while maintaining appropriate professional boundaries

Housing & Program Support

• Manage client eligibility and required documentation for HOPWA-funded services
• Support clients receiving Tenant-Based Rental Assistance, including required home visits, eligibility documentation, and ongoing program paperwork
• Maintain accurate and timely funder-required documentation and eligibility records
• Participate in program and community meetings and serve as a knowledgeable resource for the rural service area
• Develop and maintain relationships with landlords, housing providers, healthcare partners, and other community resources

Independent Field & Community Work

• Independently manage your schedule, caseload, documentation, and program responsibilities while staying connected to the larger Nashville CARES team
• Conduct client visits throughout the rural service area, including home and community-based visits
• Plan travel efficiently when serving clients across multiple counties
• Research resources and solutions when unique client needs arise and know when additional support or consultation is needed
• Coordinate closely with Nashville CARES staff and community partners to ensure clients have access to the services they need
• Travel to the Nashville CARES office approximately one day per week for team collaboration, client needs, documentation, and other agency responsibilities

• Resourceful and willing to research, make calls, and find solutions when the answer isn’t immediately available
• Patient, supportive, and adaptable when working with clients who may be facing significant barriers
• Detail-oriented and understand the importance of accurate, timely documentation
• Comfortable balancing field-based client work with administrative responsibilities
