Job summary

Nashville CARES is seeking a Rural Case Manager – Housing & Support Services based in Clarksville, TN. The role focuses on helping rural residents access housing, healthcare, transportation, nutrition, and community resources.

You will conduct independent field visits across multiple counties, manage complex cases, and ensure timely documentation while coordinating with partners and the Medical Case Management Coordinator. Bachelor’s (required) and three years’ experience are expected.

Qualifications

• Bachelor’s degree required; degree in Social Work, Human Services, or a related field preferred
• Master’s degree preferred
• Minimum of three years of experience in community-based social services, healthcare, or case management
• Proficiency with Microsoft Office and general tech; EHR training provided
• Valid driver’s license and reliable vehicle for travel across rural counties

Responsibilities

• Assess client circumstances, needs, and barriers and connect them with appropriate services
• Provide ongoing case management including assessments, care planning, and eligibility support
• Help clients access housing, nutrition, transportation, and other resources
• Assist clients with locating housing or maintaining current housing
• Travel to rural counties for home visits and program responsibilities
• Maintain accurate documentation and coordinate with Nashville CARES staff and partners
