Advocate Health is seeking a Physical Therapist for a full-time role at a small rural hospital. The position involves evaluating patients, creating and adjusting treatment plans, and guiding discharge planning. The role requires Wisconsin licensure and BLS within 6 months, with weekend coverage and additional premiums for Fri–Sun shifts.

Expect a 5-day schedule, including Friday, Saturday and Sunday, with an extra $11/hour on those days. There is a $20,000 sign-on bonus for full-time hires.

Qualifikationen

• Intermediate computer skills.
• Excellent communication and interpersonal skills.
• Demonstrated organization, decision-making, and problem solving skills.
• Team members will maintain annual educational requirements for hospital based certifications (e.g. DNV, CARF or others).

Aufgaben

• Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations.
• Establishes a plan of care and implements individual treatment based on evaluation results and functional goals.
• Monitors and evaluates outcomes and adjusts the treatment plan as needed; provides updates to physicians.
• Plans for and completes patient discharge with home programs, teaching, and referrals.
• Completes documentation including progress notes, reports, billing, and scheduling.
• Cleans up after treatment, maintains neat work area, and handles equipment safely.
• Demonstrates use of modalities and exercise equipment per protocol.
• Participates in performance improvement to enhance patient care quality.
• Provides age-appropriate care and considers development in planning.
