Job summary

Advocate Health is seeking a compassionate and motivated Physical Therapist to join our Home Health team. You will work independently in patients' homes, collaborating with physicians, nurses, and other professionals to deliver skilled therapy that improves mobility and safety.

This role emphasizes patient-centered care and meaningful family interactions. The position requires IL licensure (or eligibility), travel across the Southwest Suburbs, and a 40-hour week with a $10,000 sign-on bonus for

Qualifications

• Licensed Physical Therapist in the State of Illinois (or eligible for licensure).
• Minimum one year of clinical physical therapy experience or strong clinical affiliation experience.
• Valid driver's license and reliable transportation.
• Basic computer proficiency, including email and Microsoft Office applications.
• Strong communication, documentation, organizational, and time-management skills.
• Ability to work independently and manage a geographically assigned caseload.

Responsibilities

• Evaluate, develop, implement, and modify individualized physical therapy treatment plans for home health patients.
• Assess patient mobility, safety, functional abilities, and equipment needs within the home environment.
• Help patients progress toward optimal independence and self-care.
• Serve as a case manager and coordinate care with physicians, nurses, social workers, and other interdisciplinary team members.
• Educate patients and caregivers on treatment plans, safety strategies, and home exercise programs.
• Complete timely and accurate clinical documentation.
• Participate in care coordination activities, team meetings, and quality improvement initiatives.
