Job summary

Advocate Health at Home is seeking a licensed Physical Therapist to manage a home health caseload in the Lincoln region. You will assess patients, develop and implement a plan of care, and coordinate with physicians and a multidisciplinary team to achieve optimal outcomes.

The role requires a Doctor of Physical Therapy, an active PT license, and a valid driver’s license. The schedule is Mon-Fri 8:30–5 with weekend rotation and a sign-on bonus up to $15,000 for eligible applicants.

Qualifications

• Doctor of Physical Therapy degree required.
• Current Physical Therapy License required in applicable state.
• Valid Driver's license and auto insurance.

Responsibilities

• Manage daily patient caseload in home health with timely care and documentation.
• Collect data from assessments and EMR to determine issues and plan interventions.
• Identify high-risk patients and coordinate with multidisciplinary team to ensure quality outcomes.
• Prioritize issues based on patient health, safety, and service impact.
• Evaluate rehabilitation needs and develop plans of care with physician input.
• Lead and supervise therapy staff involved in patient care.
• Maintain communication with team on schedule changes and patient data.
• Incorporate short and long-term goals into therapy plans and progress notes.
• Collaborate with physician oversight and branch supervision for timely feedback.
• Perform treatments in accordance with PT practice acts and guidelines.
