Job summary

Equum Medical is seeking a Project Manager for the Rural Health Transformation NY-VISTA Award to lead planning, coordination, and reporting for grant-funded rural health initiatives. You will collaborate with clinical teams, government agencies, and partners to achieve program goals and maintain timelines and budgets.

Qualifications

• Bachelor's degree in Healthcare Administration, Public Health, Business Administration, Project Management, Communications, or a related field.
• Minimum of three (3) years of project management experience, preferably in healthcare, public health, rural health, community development, or grant-funded programs.
• PMP certification or equivalent credential preferred.
• Experience managing multiple projects and competing priorities.
• Experience facilitating cross-functional teams and stakeholder collaborations.
• Experience with federally or state-funded grant programs preferred.
• Knowledge of rural healthcare systems and healthcare transformation initiatives preferred.
• Experience with performance improvement and quality management methodologies.
• Strong project management and organizational skills.
• Excellent written and verbal communication abilities.
• Proficiency with Microsoft Office Suite, project management software, and data reporting tools.
• Ability to independently manage priorities and meet deadlines.
• Knowledge of healthcare regulations, grant administration, and reporting requirements.

Responsibilities

• Lead project planning, timelines, milestones, and workstreams aligned with NY-VISTA award objectives.
• Coordinate strategic initiatives across organizations and communities.
• Monitor progress, risks, and opportunities for improvement.
• Facilitate meetings, workgroups, and steering committees.
• Ensure deliverables meet grant requirements and deadlines.
• Engage healthcare providers, community organizations, and government partners.
• Develop dashboards and tools to monitor KPIs and outcomes.
• Support budget forecasting and financial reporting.
• Coordinate grant development activities and proposals.
