Job summary

MDH Worcester County Health Department seeks a full-time coordinator to implement and monitor the Rural Health Transformation Program, improving access to care and strengthening partnerships across rural communities.

You will coordinate initiatives, work with internal and external stakeholders, and ensure grant compliance while assisting with data collection, reporting and evaluation. Some travel may be required, including evenings or weekends.

Qualifications

• Experience building collaborative relationships with healthcare providers and community organizations.
• Experience coordinating multiple projects.
• Experience collecting, analyzing, and preparing reports with established deadlines.

Responsibilities

• Coordinate, implement, and monitor Rural Health Transformation Program activities.
• Collaborate with internal departments and external stakeholders.
• Monitor program performance and ensure compliance with grant requirements.
• Assist with data collection, reporting and evaluation.
• Travel and occasional evening/weekend hours may be required.
