Job summary

Maryland AG in Cumberland, MD is seeking a Health Program Coordinator to oversee rural health transformation funds and nutrition initiatives. The role includes health education, outreach, nutrition education, helping develop referral systems, and collaborating with partners.

You will track grant data for performance measures and participate in committee meetings to share input on nutrition programs, tobacco education, and related initiatives with community partners and businesses.

Qualifications

• Education: Graduation from an accredited high school or possession of a high school equivalency certificate.
• Experience: One year of experience in providing information on health care and disease prevention within the community.
• Notes: 1. Substitute experience for education on a year-for-year basis.
• Notes: 2. Substitute U.S. Armed Forces health service experience for required experience on a year-for-year basis.

Responsibilities

• Coordinate rural health transformation funds to address Built Environment Food and Nutrition Initiative.
• Provide health-related education and outreach, including nutrition education and support.
• Assist in developing referral systems and aid nutritionists as needed.
• Track and compile grant-related data for performance measures reports.
• Collaborate with community partners and businesses and attend committee meetings to discuss nutrition initiatives.
