Job summary

Mercy seeks a Rural Health Transformation Program Navigator to serve as a frontline liaison between community residents and healthcare and social service resources. You will help individuals navigate complex systems, address social determinants of health, and expand access to care across assigned counties.

In this role you will support Hub-approved community health initiatives, collaborate with local partners, and work independently in community settings to improve outcomes while offering

Qualifications

• Associate degree required; Bachelor’s preferred in Social Work, Public Health, Community Health, Human Services, or related field.
• Two years of experience in community outreach, case management, patient navigation, or social service delivery preferred.
• Strong interpersonal, communication, and problem-solving skills.
• Ability to work independently in community settings with limited supervision.

Responsibilities
