Navigator responsibilities include supporting Hub-approved initiatives and collaborating with local partners to strengthen community health outcomes and equity.

Qualifications

• Associate degree required; Bachelor’s preferred in Social Work, Public Health, Community Health, Human Services, or related field.
• Two years of experience in community outreach, case management, patient navigation, or social service delivery preferred.
• Strong interpersonal, communication, and problem-solving skills.
• Ability to work independently in community settings with limited supervision.

Responsibilities
