Job summary

Mercy is seeking a Rural Health Transformation Program Navigator to serve as a frontline liaison helping residents access healthcare and community resources. You will address social determinants of health, navigate systems, and support implementation of community health initiatives across assigned counties.

Ideal candidates have an associate degree and experience in outreach, with strong communication and problem-solving abilities to work independently in community settings.

Qualifications

• Associate degree required; Bachelor’s preferred in Social Work, Public Health, Community Health, Human Services, or related field.
• Two years of experience in community outreach, case management, patient navigation, or social service delivery preferred.
• Strong interpersonal, communication, and problem-solving skills.

Responsibilities

• Serve as frontline liaison between community residents and healthcare and social service resources.
• Support individuals in navigating complex systems and social determinants of health to improve access to care.
• Collaborate with local partners to implement Hub-approved community health initiatives.
