Job summary

Mercy Hospital St. Louis Critical Care Medicine Fellowship is seeking a Rural Health Transformation Program Navigator to serve as a frontline liaison between community residents and healthcare and social service resources.

This role supports individuals in navigating complex systems, addressing social determinants of health, and improving access to care across assigned counties. Navigators support implementation of Hub-approved community health initiatives and collaborate with local partners to

Qualifications

Associate degree required; Bachelor’s preferred in Social Work, Public Health, Community Health, Human Services, or related field.
Two years of experience in community outreach, case management, patient navigation, or social service delivery preferred.
Strong interpersonal, communication, and problem-solving skills.
Ability to work independently in community settings with limited supervision.
