Job summary

Mercy is seeking a Rural Health Transformation Program Navigator to serve as a frontline liaison between community residents and available healthcare and social service resources in assigned counties.

You will help individuals navigate complex systems, address social determinants of health, and improve access to care while supporting Hub-approved initiatives and collaborating with local partners to enhance community health outcomes.

Qualifications

• Associate degree required; Bachelor’s preferred in Social Work, Public Health, Community Health, Human Services, or related field.
• Two years of experience in community outreach, case management, patient navigation, or social service delivery preferred.
• Strong interpersonal, communication, and problem-solving skills.
• Ability to work independently in community settings with limited supervision.
