Job summary

Katy Trail Community Health in Missouri is seeking a dedicated Navigator (CHW) to join our Rural Health Transformation Program. This full-time, field-based role connects residents to clinical, behavioral, and social services through local Community Hubs and the Community Information Exchange (CIE).

You will build trust with residents, manage referrals, and ensure follow-through while traveling locally to collaborate with hospitals, FQHCs, EMS, and community partners.

Qualifications

• Formal CHW certification or related post-secondary education
• Experience working with rural/high-need communities
• Ability to document and track activities using digital systems (CIE, EHR, or case management tools)

Responsibilities

• Conduct standardized screening and intake to identify clinical, behavioral, and social needs; document findings and referrals in CIE.
• Manage closed-loop referrals and warm handoffs with Hub partners; confirm service receipt and re-engage if needed.
• Engage Medicaid/high-need residents via calls, field visits, and community outreach; provide health education and navigation.
• Help residents access hub services including primary care, behavioral health, EPSDT, and non-emergency transportation.
• Identify and resolve barriers to care; escalate complex issues to Hub Program Manager as needed.
• Own assigned caseload and maintain follow-up until services are completed or transitioned.
