Job summary

Main Street Health, the nation's leading value-based care provider serving rural America, seeks a Health Navigator in Savannah, GA. You will build trusted relationships with patients and clinics, enter data, schedule visits, and coordinate care alongside clinic staff.

This role emphasizes empathy, data entry, insurance navigation, and collaboration with a fast-paced team. Proficiency in Spanish and a proactive, independent work style are required for a 40-hour work week, Monday–Friday.

Qualifications

• Self-starter and independent worker with a strong service mentality.
• Ability to build trusting relationships with patients and providers.
• Comfortable using data to inform decisions and activities.
• Fluent in Spanish; bilingual skills required for patient interactions.
• Familiarity with multiple software platforms is beneficial.

Responsibilities

• Develop strong relationships with patients to assist them with their care
• Meet with patients during the check-in process, entering data into the Main Street software platform for reporting and tracking purposes
• Call patients to schedule an office visit or remind them to pick up their medication
• Order and schedule various procedures, tests and screenings
• Identify and track progress on important gaps in patient care
• Maintain a record of patient interactions and communicate with providers using electronic health records
• Educate patients on their healthcare options, insurance benefits,and common medicalconditions
• Help patients access various community resources
• Help coordinate follow-up care after patients have been discharged from the hospital
• Demonstrate compassion for patients, adaptability, attention to detail, patience and an eagerness to collaborate with team members
