Responsibilities

• Develop strong relationships with patients to support their care.
• Meet with patients during check-in and enter data for reporting and tracking.
• Call patients to schedule visits or remind about medications.
• Order and schedule procedures and screenings.
• Maintain records of patient interactions and communicate with providers via EHRs.
