Job summary

Allscreens Nationwide Ltd is seeking a compassionate Rough Sleeping Case Officer to join the South and East Lincolnshire Councils Partnership. You will deliver person-centred support to people sleeping rough or at risk, helping them access accommodation and sustainable housing.

The role requires street outreach, building trusted relationships, coordinating with housing providers, health professionals and support agencies, and contributing to multi-agency plans.

Qualifications

  • Experience working within housing, homelessness, support services or a related field.
  • Ability to engage positively with individuals experiencing complex needs.
  • Strong organisational skills and the ability to manage a varied caseload.
  • Willingness and ability to travel across the district and undertake outreach work.
  • Physically able to walk significant distances and work outdoors in all weather conditions.

Responsibilities

  • Conduct street outreach sessions, including some early morning work, to identify and support individuals sleeping rough.
  • Build trusted relationships with clients and help them overcome barriers to housing and independence.
  • Provide tenancy sustainment support to help people maintain accommodation.
  • Develop personalised support plans tailored to individual needs and aspirations.
  • Complete assessments, risk management plans and safeguarding referrals.
  • Work collaboratively with landlords, partner agencies and statutory services.
  • Contribute to multi-agency meetings and coordinated support plans.
  • Activate SWEP during severe weather periods with some on-call hours.

Our client is looking for a compassionate, motivated and resilient Rough Sleeping Case Officer to join their dedicated Rough Sleeping Prevention and Recovery Team. This rewarding role offers the chance to work alongside individuals experiencing homelessness, helping them move from crisis and uncertainty towards safety, stability and independence.

As a Rough Sleeping Case Officer, you'll be at the heart of their efforts to prevent and reduce rough sleeping across the district.

You'll provide practical, person-centred support to people who are:

  • Sleeping rough or at immediate risk of rough sleeping.
  • Living in temporary or emergency accommodation.
  • Facing complex challenges such as poor mental health, substance misuse, trauma or social isolation.

Every day is different. One moment you could be conducting outreach to engage with someone sleeping rough, and the next helping a client secure accommodation, reconnect with services, or develop the skills and confidence they need to maintain a tenancy.

You'll manage your own caseload while working closely with housing providers, health professionals, support agencies and community organisations to deliver positive, long-lasting outcomes.

What You'll Be Doing:

  • Contributing to multi-agency meetings and coordinated support plans.
  • Activating SWEP, Severe Weather Emergency Protocol, which includes some evening, early morning and weekend work during a short period of time on a rota basis.

This role requires flexibility, including occasional early morning, evening and weekend work, particularly during periods of Severe Weather Emergency Protocol (SWEP) activation. The successful applicant will be required to undertake a DBS check.
