Job summary

Humbercare Ltd is seeking a dedicated Supported Living Worker to join our HRS team. This role involves delivering person-centred support to customers, promoting independence, and ensuring safe, compassionate service across properties.

You will engage weekly with clients, develop and implement care and risk management plans, and act as a trusted ambassador for Humbercare, coordinating with local resources to improve quality of life.

Qualifications

  • Educated to Level 2 Diploma or equivalent.
  • Experience of working in a social care or similar environment.
  • Knowledge of statutory and voluntary support agencies.
  • Working knowledge of person-centred, strengths-based approaches to support and care planning.
  • Experience of working with others to achieve successful outcomes.
  • Excellent communication, interpersonal and problem-solving skills.
  • Team working experience, working cooperatively with colleagues in complex, pressurised situations.
  • Excellent organisational and time management skills and the ability to effectively manage a caseload.
  • The successful candidate(s) will hold a current driving licence and have access to transport, as travelling between properties and on occasion transporting clients would be essential to the role.

  • To engage with customers on a weekly basis to support individuals to secure, maintain and achieve independent living in the community.
  • The co-provision of support plans, risk management plans, crisis intervention and engaging with customers in order to deliver positive outcomes in a service that offers involvement and empowerment for customers.
  • To provide a high-quality service; ensuring outstanding customer service and service delivery by acting as an ambassador for Humbercare and point of contact for customers. Engaging with customers to design services that work for them and utilising resources within the local area to make a real, quality difference to peoples' lives

Contract
  • 40 hrs per week, Full Time, Permanent
