Job summary

FPACP Houston LLC seeks a licensed Social Worker to join our team in Houston, TX. The role relies on knowledge of community resources and professional judgment to refer residents to services and promote social well‑being for residents and families.

The position requires a TX license (or within 6 months of hire), a minimum of 2 years in long-term care or related settings, and the ability to respond to emergencies as needed. A supportive team and RockStar recognition are highlighted in our culture.

Qualifications

• Bachelor's Degree in Social Work or related field.
• Licensed Social Worker in Texas or to obtain within 6 months of hire.
• Prefer 2 years' experience in long-term care or related medical facilities.
• Ability to respond appropriately in emergencies.

Responsibilities

• Meet with administration, medical and nursing staff to plan social services.
• Advocate daily on behalf of residents to protect their rights.
• Report abuse, neglect, or exploitation per state guidelines.
• Complete Social Service History and Evaluation for new residents within 14 days.
• Complete MDS 3.0 sections on schedule and changes.
• Educate residents on Advance Directives and Power of Attorney.
• Facilitate referrals to ancillary services and coordinate orders.
• Assist with end-of-life care decisions and transitions.
• Prepare social service evaluations prior to care plan updates.
• Maintain intradepartment rapport and community resources.
