Job summary

Focused Post Acute Care Partners (FPACP) is seeking an experienced Social Worker in Gilmer, TX. You will interview, refer, and coordinate resources to meet residents’ social and emotional needs, working with a diverse team to support the well-being of patients.

You will lead planning meetings, advocate for residents, and ensure compliance with regulatory requirements while educating families about care options and advance directives.

Qualifications

• Bachelor's Degree in Social Work or a Human Services field.
• Must be a licensed Social Worker in Texas or obtain licensure within 6 months of hire.
• Prefer two years of experience in long-term care, hospital, or related medical facilities.
• Able to react to emergency situations appropriately when required.

Responsibilities

• Meet with administration, medical and nursing staff, and other related departments to plan social services.
• Advocate daily on behalf of residents to ensure rights are maintained.
• Report abuse, neglect, or exploitation per state guidelines.
• Maintain professional rapport with interdisciplinary team and community resources.
• Complete Social Service History and Evaluation for new residents within 14 days.
• Complete sections of MDS 3.0 on specified days.
• Educate and assist with Advance Directives, Medical Power of Attorney, and DNR documents.
• Coordinate referrals to ancillary services (Optometry, Audiology, Dental, Podiatry, Counseling, Psychiatry, psychology).
• Educate residents/families about Palliative vs Hospice care and assist in transitions to end-of-life services.
• Assist with Medicaid ambulance transport authorizations and transportation scheduling.
• Prepare Social Service Evaluation for each resident's care plan.
