Focused Post Acute Care Partners is seeking a licensed Social Worker to join our team in a Texas long-term care setting. You will interview, coordinate and refer residents to resources, and promote their social and emotional well-being.

You will advocate daily for residents' rights, complete required assessments, assist with advance directives and end-of-life decisions, and coordinate with families and community resources.

Competenze

• Must be a licensed Social Worker in the state of Texas or obtain within 6 months of hire.
• Bachelor's degree in Social Work or related Human Services field.
• Prefer two years of experience in long-term care, hospital, or related facilities.
• Able to react to emergency situations appropriately when required.

Mansioni

• Meet with administration, medical and nursing staff, and related departments to plan social services.
• Advocate daily on behalf of residents to ensure their rights are maintained.
• Complete Social Service History and Social Service Evaluation within 14 days.
• Assist with care planning, DNR, and advance directives for residents.
• Facilitate referrals to ancillary services and coordinate with families.
• Maintain knowledge of federal and state long-term care regulations.
