Job summary

Hampton Court Nursing and Rehabilitation Center is seeking a dedicated Wound Care Nurse to join our skilled nursing team. The RN will assess, treat, and monitor residents with wounds, promoting healing and preventing complications while ensuring compliance with standards and regulations.

Qualifications

• Current RN license in the state.
• Wound care certification preferred or willingness to obtain within timeframe.
• Minimum of three years of nursing experience; skilled nursing/long-term care preferred.
• Experience in wound care management and pressure injury prevention.

Responsibilities

• Perform comprehensive skin and wound assessments on admission and routinely thereafter.
• Develop, implement, and evaluate individualized wound care plans.
• Provide evidence-based wound care, including dressing changes and advanced therapies per physician orders.
• Monitor healing progress and revise plans as needed.
• Educate residents and families on wound prevention and skin integrity.
• Document wound assessments and treatments accurately in the EMR.
• Collaborate with the interdisciplinary team to optimize outcomes.
• Support CMS and regulatory compliance in wound care practices.
