Job summary

Your Health Organization in Georgia is seeking a Registered Nurse to join our growing primary care family as a Visiting Nurse Case Manager. The role requires visiting patients in homes and facilities, coordinating with providers, and initiating telemedicine visits.

Travel is a daily expectation to service the Tifton/Moultrie area and surrounding facilities. The ideal candidate is a licensed RN with strong communication skills, a collaborative mindset, and the ability to manage diverse care plans

Qualifications

• Must be a licensed nurse.
• Experience in community settings preferred.
• Strong written and verbal communication skills.
• Ability to manage multiple responsibilities with sound judgment.
• Willingness to travel between offices and care facilities.

Responsibilities

• Facilitate receiving medical records from patients' providers and specialists.
• Enroll patients in Care Management if eligible; initiate Care Management Plan of Care.
• Coordinate and communicate with interdisciplinary care teams and caregivers.
• Provide post-discharge education and evaluate home health needs.
• Travel to patients' homes and care facilities as required.
