Job summary
Jobtailor is seeking a dedicated RN to support hospitalized patients with care coordination and discharge planning using virtual technology. The role operates from Virtua’s centralized command center and collaborates with patients, families, and interdisciplinary teams to ensure safe transitions from hospital to home.
The ideal candidate has a NJ RN license, 3+ years of acute care experience, and strong communication, assessment, and clinical judgment skills to support medication reconciliation
Qualifications
- Graduate of an accredited nursing program.
- Current State of New Jersey nursing license to practice as a registered nurse.
- Current CPR certification BLS.
- Ability to maintain competency in required virtual care technologies.
Responsibilities
- Support hospitalized patients through care coordination and discharge planning during acute care stay using virtual technology.
- Operate from Virtua’s centralized command center and collaborate with patients and interdisciplinary teams.
- Review clinical information and identify barriers to discharge and ensure medication accuracy.
