Software Placement Group in Connecticut seeks a clinically strong Unit Manager (RN) to oversee two downstairs long-term care and skilled nursing units. Reporting to the Director of Nursing, this leader holds 24/7 clinical and administrative accountability for their units, ensuring high-quality resident care and full regulatory compliance.

The role requires leadership, LTC regulatory expertise, and hands-on oversight of care plans, rounds, MDS assessments, wound management, and readmission

Qualifications

• Active RN license in CT.
• 3+ years clinical nursing in SNF/LTC.
• 1–2 years in a formal leadership role.
• Deep LTC regulatory expertise and DPH survey prep.
• EMR experience with PointClickCare for care management.

Responsibilities

• Oversee two downstairs resident care units and supervise RNs, LPNs, and CNAs.
• Ensure compliance with Connecticut DPH regulations, CMS guidelines, and QAPI metrics.
• Manage care plans, physician rounds, MDS assessments, wound management, and readmission prevention.
• Mentor staff and coordinate with the Daytime RN Desk Nurse/Assistant Unit Manager to balance workflows.
• Serve as primary escalation point for physicians, providers, residents, and families.
• Conduct root-cause analyses for falls, medication errors, and skin issues; implement corrective actions.
