Qualifications

• Licensed to practice as an RN in Connecticut.
• Prior long-term care experience required.
• Leadership experience in a long-term care setting.

Responsibilities

• Care Needs & Planning: Develop, implement and update individual care plans; support hands-on care.
• Assessments: Complete admissions, discharges and other assessments as needed.
• Triage & Interventions: Address acute health changes with physician orders; collaborate with care providers & families.
• Regulatory Standards: Ensure compliance with federal, state, county & company health/safety regulations.
• Quality Assurance: Audit documentation & care for quality control/compliance.
• Family & Team Liaison: Communicate resident changes to families/providers; work with the interdisciplinary team.
• Team Oversight & Mentorship: Provide training, orientation & evaluations; manage staffing schedules.
