Job summary

Focus Health Network's Milton Nursing & Rehab seeks an experienced RN Unit Manager to lead the clinical team, overseeing daily nursing operations, ensuring compliance with federal and state regulations, and guiding LPNs and CNAs to deliver resident-centered care.

You will mentor staff, develop individualized care plans, conduct assessments, and communicate with physicians and families about changes in condition.

Qualifications

• Active PA RN license.
• 2–3 years of nursing experience in SNF/LTC.
• Leadership experience preferred.
• Strong knowledge of MDS and state/federal regulations.

Responsibilities

• Clinical Leadership: supervise and evaluate unit nursing staff to ensure high‑quality care and adherence to care plans.
• Resident Care: oversee development and implementation of individualized care plans and regular assessments.
• Compliance & Safety: ensure compliance with applicable laws, regulations, and facility policies.
• Family Communication: communicate with physicians, families, and residents regarding care options and changes in condition.
• Quality Assurance: participate in wound management, infection control, and incident investigations.

Position Summary:

We are seeking a dedicated, energetic, and experienced Registered Nurse (RN) Unit Manager to lead our clinical team. You will be responsible for overseeing the daily operations of our nursing unit, ensuring compliance with federal and state regulations, and guiding a team of nurses (LPNs, CNAs) to provide the highest quality care.

Key Responsibilities:

• Clinical Leadership: Supervise, mentor, and evaluate unit nursing staff to ensure high-quality care and adherence to care plans.
• Resident Care: Oversee the development and implementation of individualized care plans, conducting regular assessments.
• Compliance & Safety: Ensure compliance with all applicable laws, regulations, and facility policies.
• Family Communication: Communicate effectively with physicians, families, and residents regarding care options and changes in condition.
• Quality Assurance: Participate in wound management, infection control, and incident investigations.

Requirements:

• Current, active PA state license as a Registered Nurse (RN).
• Minimum 2-3 years of nursing experience, preferably in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting.
• Previous leadership experience (e.g., Charge Nurse, Supervisor) is a plus.
• Strong knowledge of MDS, state/federal regulations, and care planning.
• Exceptional communication and team-building skills.
