Job summary

Life Care Centers of America is seeking an RN Unit Care Coordinator responsible for supervising, coordinating, and managing patient care on the unit. You will work closely with patients, families, and nursing staff to uphold quality standards and regulatory compliance.

The role requires an RN license, a nursing diploma (ADN or BSN), and pediatric or geriatric experience is preferred. CPR must be current. Expect on-site scheduling and shared on-call duties as part of the position.

Qualifications

• Nursing diploma (associate's or bachelor's degree in nursing).
• Currently licensed/registered in applicable State. Must maintain an active RN license.
• One year geriatric nursing experience preferred.
• CPR certification upon hire or during orientation and kept current.

Responsibilities

• Direct daily functions of unit nurses and CNAs to provide floor leadership.
• Chart appropriately and timely.
• Provide, manage, and coordinate patient care and services to attain highest practicable well-being.
• Accurately prepare and administer medication as ordered by a physician.
• Respond in leadership capacity to emergencies related to patient and staff safety.
• Coordinate patient care plans and services.
• Exhibit excellent customer service and maintain a positive attitude toward patients.
• Demonstrate reliable attendance.
• Communicate effectively within interdisciplinary teams.
