Job summary

Fidelis Care - New York is seeking a field-based care manager to coordinate complex/high acuity member care in MLTC programs. The role requires NY RN licensure (or NP) and 4–6 years of related experience, with a focus on person-centered planning and family education.

You will collaborate with a care management team, assess needs, and connect members with providers, community resources, and services while ensuring regulatory compliance and high-quality care delivery.

Qualifications

• Requires RN or NP licensure with nursing degree.
• 4–6 years of related care management experience.
• Graduate from an accredited nursing program or BSN preferred.

Responsibilities

• Performs care management duties for complex/high acuity populations.
• Develops personalized care plans and educates members and families.
• Coordinates with providers and community resources for needed services.
• Monitors and revises care plans to meet member goals and needs.
• Ensures compliance with state, federal, and clinical guidelines.
• Provides education and acts as liaison between member, family, and providers.
• May perform home visits and participate in onboarding of new hires.
