1st Choice, LLC is seeking a Registered Nurse UAS Assessor for Canarsie, NY. The role offers an independent schedule with a hybrid mix of in-home and telehealth assessments, enabling nurses to deliver meaningful clinical impact while maintaining work-life balance.

Ideal candidates have 1–2 years of RN experience in one-on-one in-home, community, or geriatric care, with an active NY RN license and a BSN. Mileage-based travel and NYS DOH compliance are part of this role.

Qualifications

• Must have own car for daily travel between assessment locations
• RN license valid in New York
• BSN is required
• 1–2 years of RN in home health, community, or geriatric care

Responsibilities

• Complete UAS NY assessments during in home visits or via telehealth
• Review medical documentation and record accurate detailed findings
• Educate consumers and communicate with providers when needed
• Maintain timely high quality documentation that meets NYS standards
• Manage your schedule, route planning and daily workflow
• Follow all conflict free requirements under the NYS DOH
