1st Choice, LLC is seeking a Registered Nurse UAS Assessor for the Midwood-Flatbush area of New York. This independent, hybrid role offers flexible scheduling and meaningful clinical impact as you perform in-home and telehealth assessments.

You will review medical documentation, document findings, and coordinate with providers while meeting NYS standards. A valid NY RN license and BSN are required, with 1–2 years home health experience preferred.

Qualifications

• Active RN license in NY.
• 1–2 years of RN home health experience.
• BSN preferred, experience with UAS-NY is a plus.
• Strong communication and time management skills.
• Proficiency with EHR and accurate documentation.

Responsabilités

• Complete UAS NY assessments during in home visits or via telehealth.
• Review medical documentation and record accurate, detailed findings.
• Educate consumers and communicate with providers when needed.
• Maintain timely high-quality documentation meeting NYS standards.
• Manage your schedule, route planning, and daily workflow.
• Follow conflict-free requirements under NYS DOH.
