Job summary

Central Ohio Primary Care Physicians, Inc. in Westerville, OH is seeking a Triage Specialist-RN (Day Shift) to conduct telephone nursing triage and coordinate patient care within a care-navigation team.

The role requires a BSN, an active Ohio RN license, and 2+ years of RN experience in hospital or medical practice settings. Telephonic triage experience is preferred; hybrid work schedule with standard clinic hours.

Qualifications

• Bachelor's degree in Nursing required.
• Active and unrestricted RN license in Ohio required.
• Minimum 2 years RN experience in hospital or medical practice.
• Telephonic nursing triage experience preferred.

Responsibilities

• Conduct thorough telephonic nursing triage for acute and chronic medical complaints.
• Navigate multiple electronic platforms to assess health concerns, chart, and schedule appointments.
• Document nursing triage notes in the EHR for clear documentation.
• Document call details for department data tracking and reporting.
• Identify and address patient care gaps.
• Collaborate with COPC staff, offices, and physicians to deliver high-quality care.
