Responsibilities

• Coordinate medical, surgical, financial, psychosocial, nutrition, and pharmaceutical care for transplant candidates/recipients and living donors.
• Educate patients and families and maintain open communication with the care team and external providers.
• Document care in EMR/OTTR and ensure HIPAA-compliant record keeping.
