Qualifications

• Associates degree or nursing diploma required; Bachelor's degree preferred.
• Minimum 3 years of nursing experience.

Responsibilities

• Facilitates medical, surgical, financial, psychosocial, nutrition and pharmaceutical evaluation and ongoing care for transplant patients and living donors.
• Educates patients and families about the transplant process and care plans.
• Acts as a case manager across all phases of transplantation with accurate documentation.
• Communicates within the multidisciplinary team to align plans of care.
• Participates in quality monitoring, process improvement and regulatory compliance.
• Complies with Wellstar Health System policies and code of conduct.
