Job summary

Wellstar Health System is seeking a transplant coordinator RN to coordinate care for transplant patients, living donors, and families. You will educate, support, and coordinate across the transplant continuum, ensuring aligned plan of care and timely documentation.

The role requires clinical leadership in education, patient advocacy, and program development within a multidisciplinary team, with emphasis on quality improvement and regulatory compliance.

Qualifications

• Associates degree or nursing diploma; Bachelor's in Nursing preferred.
• RN-compact license or single-state RN license.
• Minimum 3 years of nursing experience.

Responsibilities

• Coordinate transplant patient care across pre-, peri-, and post-transplant phases.
• Educate patients and families about the process and plan of care.
• Facilitate communication among the multidisciplinary team and care planning.
• Maintain HIPAA-compliant documentation in EMR/OTTR and regulatory standards.
• Participate in quality monitoring and process improvement activities.
