Responsibilities

• Coordinate care for transplant patients, living donors and families across the care continuum.
• Educate patients and families about the transplantation process and care plan.
• Act as a liaison within the multidisciplinary team and care settings to ensure the plan of care.
• Maintain accurate documentation in EMR/OTTR and ensure HIPAA compliance.
• Support performance improvement initiatives within the transplant program.
