Responsibilities

• Patient Care Coordination across transplant candidates/recipients and living donor patients through all phases of transplantation.
• Education to patients and families regarding the process and maintaining open communication with all parties.
• Documentation of care and updates in OTTR and EMR; compliance with HIPAA and regulatory standards.
• Participation in quality monitoring, process improvement activities, and program policy adherence.
