Job summary

Healthier Mississippi People (HMP) is seeking a transplant coordinator to oversee solid organ transplantation processes for pediatric and adult recipients. You will work with a multidisciplinary team across pre-transplant, transplant admission and post-transplant care, educating patients and families along the continuum.

Qualifications

• RN license and at least one year of clinical experience.
• BSN preferred; CCTC and ACLS are advantageous.
• Strong critical thinking, decision making and education skills.

Responsibilities

• Provides daily clinical assessment of transplant patients across all care phases.
• Coordinates pre-transplant evaluation to optimize hospital resources.
• Assesses post-transplant function and plans interventions as needed.
• Facilitates scheduling of studies and procedures with fiscal responsibility.
• Develops and implements care protocols with the multidisciplinary team.
• Educates recipients, donors and families throughout transplantation care.
• Monitors care quality and participates in performance improvement.
• Provides on-call coverage and coordinates donor-related activities.
• Maintains transplant records and ensures CMS/UNOS regulatory compliance.
