The coordinator will conduct clinical assessments, manage scheduling, ensure regulatory compliance, and provide on-call coverage to support timely transplant processes and optimal outcomes.

Responsibilities

• Provides daily clinical assessment of transplant patients across all care phases.
• Coordinates pre-transplant evaluations to optimize use of hospital resources.
• Assesses post-transplant function and coordinates follow-up care.
• Facilitates scheduling of required studies and procedures with cost awareness.
• Develops and implements care protocols in collaboration with the team.
• Educates patients and families about transplantation, organ donation, and care plans.
• Maintains regulatory compliance per CMS and UNOS requirements.
• Communicates with the multidisciplinary team and families in a timely manner.
