Job summary

UC San Diego Health is seeking a CLIN NURSE 2 for the Heart Transplant department. The role supports pre-heart transplant coordination, education, and care coordination across multiple sites including La Jolla, Hillcrest, and East Campus, with travel as needed.

Responsibilities include coordinating evaluations, waitlist management, and serving as a liaison among patients, families, and the multidisciplinary team. An RN license in California and BART/BLS at hire are required.

Qualifications

• Must be a graduate from an accredited school of Nursing.
• Registered Nurse (RN) issued by the state of California.

Responsibilities

• Coordinate patient care for the heart transplant across all campuses throughout the transplant continuum.
• Collaborate with stakeholders to facilitate outside hospital transfer process upon referral acceptance.
• Provide clinical oversight and escalation of patient changes or care gaps to transplant stakeholders.
• Navigate Phoenix and Epic to access and document patient charts accordingly.
