Job summary

Residential Home Health and Hospice in Northbrook, IL is seeking a compassionate Transitional Nurse Liaison to join our care team. You will partner with field staff to assess hospice needs, coordinate start-of-care visits, and guide patients and families through end-of-life care with clinical excellence.

The role requires an associates degree in nursing and RN experience, including acute care or oncology backgrounds; proficiency with EPIC, HCHB, and Microsoft Office, with strong computer skills

Qualifications

• Associate degree in Nursing required.
• Minimum two years RN experience with acute care or oncology preferred.
• Proficiency with EPIC, HCHB, and Microsoft Office helpful.

Responsibilities

• Complete an initial, comprehensive assessment of patient and family to determine hospice needs.
• Obtain complete demographic information and medical history including diagnosis and PCP.
• Introduce the Agency, explain services and coordinate start-of-care visit.
• Order requested DME, supplies, and medications related to hospice care.
• Deliver community education for facilities, skilled nursing facilities, and hospitals.
• Provide patient education at bedside about hospice, progression, and end-of-life care.
• Attend discharge planning meetings with facility team to ensure safe discharge home.
• Determine hospice eligibility and appropriate level of care.
• Communicate with the clinical team on complex patients to prevent re-hospitalizations.
