MGA Homecare in Raleigh, NC seeks a Transitional Care Manager (RN) to coordinate private duty nursing efforts and strengthen partnerships with local healthcare providers. The role emphasizes facilitating smooth transitions from referral to home-based care and expanding access to personalized nursing services.

Formación

• Active RN license in good standing.
• Clinical nursing experience, especially pediatric/home health.
• Understanding of medically complex home care needs.
• Strong communication and networking skills.
• Willingness to travel throughout Raleigh.

Responsabilidades

• Develop relationships with pediatricians, hospitals, case managers, discharge planners, clinics, and other referral partners in Raleigh area.
• Identify opportunities to expand private duty nursing services and connect more families with in-home care.
• Educate healthcare providers and families about MGA’s private duty nursing services and benefits of one-on-one care.
• Use clinical knowledge to assess patient needs and identify appropriate homecare opportunities.
• Conduct in-person visits, presentations, networking, and community outreach to strengthen referral partnerships.
• Collaborate with clinical and operations teams to support transitions from referral to patient admission.
• Maintain relationships with referral partners, track outreach and referral activity, and identify opportunities for continued growth.
• Represent MGA Homecare professionally within the Raleigh healthcare community.
