Residential Home Health and Hospice is seeking a Hospice Transitional Nurse Liaison to partner with field staff and leaders to support patients and families with compassionate, team-based care.

Qualifications

• Associate degree from an accredited School of Nursing.
• RN license in good standing.
• Minimum 2 years of RN experience, including acute care or oncology; hospice experience preferred.
• Experience with Microsoft Office Suite, EPIC, and HCHB.

Responsibilities

• Complete an initial, comprehensive assessment of patient and family to determine hospice needs.
• Obtain demographic information and medical history.
• Introduce the Agency, explain scope of services, and coordinate start-of-care visit.
• Order DME, supplies, and medications related to hospice care.
• Deliver community education for facilities and hospitals.
• Provide patient education at bedside regarding hospice and end-of-life care.
• Attend discharge planning meetings with facility teams.
• Determine hospice eligibility and level of care.
• Communicate with the clinical team to prevent re-hospitalizations.
