Job summary

Conemaugh Memorial Medical Center is seeking an experienced Registered Nurse (RN) for the Transitional Care Unit (TCU). This 30-bed, hospital-based unit provides short-term rehabilitation with daily skilled nursing and therapy services to help patients return home safely.

Expect weekend rotation and no on-call duties. The ideal candidate holds an active PA RN license and a BSN, with BLS and CPR certifications.

Qualifications

• Active Pennsylvania RN license required.
• Bachelor's degree from an accredited nursing school.
• Basic Life Support (BLS) certification required within 30 days of hire.
• CPR Certification.
• Two years of nursing experience preferred.
• One year of long-term care experience preferred.
• LTC experience preferred.

Responsibilities

• Assess needs of residents and establish priorities for resident care.
• Reassess and evaluate changes in resident's condition and document changes.
• Initiate and monitor IV therapy and administer medications.
• Document data in residents' medical records and follow care plans.
• Provide teaching to residents and families based on standardized protocols.
• Supervise staff in the absence of the DON, including scheduling and performance evaluation.
• Maintain confidentiality of resident information.
