Job summary

Conemaugh Memorial Medical Center, part of Lifepoint Health, is seeking a Registered Nurse (RN) to join our Transitional Care Unit (TCU) in Johnstown, PA. You’ll provide skilled nursing care, coordinate therapies, and support safe patient transitions home.

Qualifications

• Active Pennsylvania RN license (single-state or compact)
• Bachelor's degree in Nursing from an accredited program
• Basic Life Support (BLS) required within 30 days of hire
• CPR Certification is required
• Two years of nursing experience preferred
• One year of long-term care experience preferred
• LTC experience preferred

Responsibilities

• Assess residents' needs and set priorities for care based on medical and nursing diagnoses.
• Reassess and document changes in resident condition.
• Administer medications following policies and regulations.
• Initiate and monitor IV therapy.
• Provide education to residents and families based on care plans.
• Maintain confidentiality and accurate medical records.
