Job Overview
Registered Nurse (RN) – Transitional Care Unit (TCU) at Conemaugh Memorial Medical Center, part of Lifepoint Health. PRN/variable 8- and 12-hour shifts with rotating weekends and holidays.
About the Team and Unit
TCU is a 30-bed short-term skilled hospital-based rehab unit. Patients receive daily nursing care and a minimum of two hours daily of PT, OT, and SLP. The unit serves post-op care, wound care, teaching/training for new diagnoses, IV therapy, pain, and chronic illness management.
Responsibilities
- Assesses residents' needs and establishes priorities for care based on medical and nursing diagnoses.
- Reassesses and evaluates changes in residents' conditions, reporting and documenting these changes.
- Assists with completion of the Minimum Data Set (MDS).
- Assesses learning needs of residents and/or families and provides teaching based on standardized protocols aligned with the plan of care.
- Evalues outcomes of teaching and documents relevant information in the resident’s medical record.
- In the absence of the Director of Nursing, provides supervisory responsibilities including discipline, assignments, scheduling, and evaluation of job performance.
- Maintains confidentiality regarding resident information.
- Accurately initiates and transcribes physicians’ orders in a timely manner.
- Administers medications in accordance with nursing policies and all applicable regulations.
- Initiates and monitors IV therapy and related responsibilities.
- Participates in reviewing and revising unit philosophies, objectives, policies, and procedures.
- Follows care plans to ensure resident care needs are met.
- Demonstrates ability to prioritize and organize work with flexibility, completing assigned work during shifts in a timely, accurate manner.
- Establishes a plan of care on new admissions and updates it as residents’ needs or conditions change.
Qualifications
- Current state RN license and a bachelor’s degree from an accredited nursing school.
- Basic Life Support (BLS) certification required within 30 days of hire.
- CPR certification.
- Preferred: 2 years of nursing experience.
- Preferred: 1 year of long-term care experience.
